您好！我园2015年春季报名入学规划现正式启动，现对适龄入学儿童入学流程做如下安排：

【招生对象】

满2周岁至6岁适龄幼儿

【报名方式】

（周二-周五）上午9:00-11:30,携幼儿亲临本园参观登记报名（平日可先致电预约）

【报名程序】

（1）在幼儿园财务室处领取《入园申请表》并详细填写。

（2）面谈

（3）通知录取，提交资料；

（4）缴费入学

【报名提交资料】

1、《幼儿保健手册》：在中山市内医院有近三个月幼儿体检报告，镇级以上医院防保科可以办理，体检当天幼儿需空腹抽血检查；
2、幼儿预防接种证明：凭接种登记本到医院防保科开具证明，如小孩在境外接受育苗接种，则需提供境外医院接种登记记录复印件；
3、幼儿有效证件：（父、母及幼儿）户口本或身份证或护照、台胞证的复印件，如果是中山市户籍儿童需要提供户主信息页，如孩子尚未登记入户则需要提供出生证；
4、幼儿一寸彩照5张，生活照（全家福）1张。

Hello! The admission of the new school-age children officially begins now. Kindly make the following arrangements for the school-age children enrollment process;

Recruitment of the new students

School-age children with the age range of 2 to 6 years old.

Registration form

The registration and admission time begins from Tuesday – Friday at 9:00am - 11:30am. (Normally you can call to make an appointment first)

Registration

1. Go to the kindergarten’s Financial room to receive "an application form" and fill in the details.

2. The school orientation process.

3. Give the personal information details.

4. Pay the tuition fees.

Application submission information

(1). Child Care Manual: The newly admitted children must be taken to any hospital in Zhongshan City for medical (blood) check-up. And the copies of the children check-up report must be submitted to the school for proof, therefore, the children are advised not to eat anything before the medical check-up on that day.

(2). Early Childhood Vaccination Certificate: the strength of the vaccination is issued based on the prevention and care of hospital certificate, In case of overseas check-up, the copies of the medical check-up forms must be submitted to the school.

(3). Children's Parental valid certificate (father, mother): The permanent registered residents of Zhongshan must provide the copies of identity card or passport or Taiwan resident’s certificate. if it not in Zhongshan City census register, the parent must provide residence information details, incase the children are not registered residents must provide a birth certificate;

(4). Children’s One-inch 5 Passport colored photos and one family picture.
